RISE-PA is a resource and referral tool that will assist individuals and families to identify needs related to the range of Social Determinants of Health categories (food insecurity, healthcare-medical access and affordability, housing, transportation, childcare, employment, utilities clothing, and financial strain), provide basic information that will assist community-based providers in determining eligibility, provide information to assist in matching services to what users are looking for, and allow providers to see other community-based services that users are registered to receive. This tool is intended to supplement in-person interactions between social service organizations and clients and will assist to ease access to services and supports beyond normal business hours. The platform is designed to support fundamental goals relative to ODP’s Everyday Lives Recommendation #8, Simplify the System as it looks to improve individual and family access to information and community services. Core information collected through RISE-PA will also be used to provide some information that will be used in the LifeCourse process and shared with the ODP Case Management System that will be developed. RISE-PA will support the common objective across all of our work to share information across systems.
RISE-PA will also assist in addressing some of the options identified in the DHS Simplify the System Project final report, i.e., providing individuals and families with support to navigate service systems and find community supports, investing in technologies such as mobile apps, and increase individual/family and provider communications.
